The first & second statements are linked thus:

If the data that you are processing can't identify a living person it is not
personal data. If you are not processing personal data then you do not fall
under this act. If the user can be identified, I would imagine those in a
small community or perhaps those who use your mobile service it is personal
data. 

If you are not processing any personal data the user has little or no rights
to that data.

Richard, 
I would agree that someone's name in isolation tells you nothing about them,
but I think your second statement is wrong. Seeing a name in a particular
context - eg the membership list of an organisation - does. Seeing it
attached to a particular book tells you what subjects they are interested
in. There are many situations where the reader might find this undesirable.

The act states that data is information relating to a living person that can

Hi, 

When a book is returned at our counter we try and shield the hold slip 
whilst writing the persons name down who has requested the 
book so that the person returning the book does not see this information. 
The hold shelf itself is behind the counter and at an 
angle, so names cannot be seen. 
The hold shelf has always been behind the counter and out of view, however 
we did remind staff to be extra careful when writing 
down names on hold slips when we had a data protection issue ourselves.

reservations by customers in self service library systems. 
In your library do you cover the book or item so that customers cannot see 
titles or details of other peoples reservations or do 
you leave the item in full view so that anyone can see what has been
