Can you double check your ref as I can't find any mention on p730 of this procedure, although it may be because I have the 2nd edition in front of me. According to the blurb 1st ed was 1992 not 1994

Subject: Page from book
request
Hi all,
Would anyone be able to fax me a copy of a page from a book? It's from Burnand & Young (eds). "THE New Aird's Companion in Surgical Studies" Churchill Livingstone, 1994 p. 730. Fax no. below if you can help.
I'm specifically after a reference which states that Bjork flap tracheostomies are not to be performed.
Many thanks
