I, personally, am all in favour of extending ILL agreements as widely as possible.
 
As a specialist trust, we find that most of the journals requested that are not held by us are not supplied by the NCC and are not available in libraries within our network. (By the way, our users are very poor at downloading articles provided by NCC for themselves.) Consequently, last year, approximately 25% of the journal articles we supplied to our users were obtained from the British Library. (We do not currently keep a record of the number of items we were unable to supply because we could find no locations within any participating networks.)
 
We already pay certain libraries for ILLs (they charge less than the BL), so adding a few more would not be a problem - it could only help.
 
In the absence of regional agreements, we have made private arrangements with individual libraries for ILLs outside our area, sometimes free, sometimes on a fee-paying basis.
 
Interestingly, I believe that the national trend has been for a significant reduction in ILL requests since the advent of the NCC. Last year compared with 2003-4, we requested only 3% fewer articles from other libraries, but supplied 10% more to them.
 
So, since any nationally arranged network seems to be somewhere in the distant future, let's devise our own national network and save NLH the bother.

As a follow on from this question, there are a lot of regional library networks [Northern & Yorkshire; WISH; SWICE etc] whose catalogues are available to search over the internet. I would be very interested in knowing if any of the NHS regional networks, including HLN and London Links mentioned below, would consider an application for membership from another library OUTSIDE of their geographical area?  With a fee charged?
 
The BMA, RCS, PLCS and NULJ are nationally available and have subscription fees. Might a regional network [here in East Anglia we have our Eden network which covers 3 WDD areas libraries]  income generate in a similar way? 
 
In advance of any decision about a national document delivery service [which I think is a long time away]  maybe there's some mileage in discussing interim, between library networks, arrangements. 
 
What do others think?
