I'd add that individual GPs and their Practices and Practice computer
systems have a very good record on keeping medical records confidential,
while they have control of them.

Nobody else is as good at it - for bulk records or access to an
identified individual.

keeping the information in the Practice, or the hospital, and
considering each request to access it is likely to fail gracefully.  A
mistake with a centralised system is likely to fail hugely when it
fails.
