The county council that I work for do not refer employees for counselling. 
The employee contacts  the counsellor and the appt is made between them. It is considered that counselling will only work if the employee is happy to try it not if the employer does the referring.  If someone fails to keep an appt. it is up to the counsellor to decide if she wishes to offer another appt.  It would depend on the reason for the failure to keep the appt.
The system seems to work best when everything is kept at a fairly relaxed level. There certainly does not seem to be a lack of clients.  I do find that folk seem to like me to suggest going for counselling, almost like giving them permission I suppose.
The service is totally confidential and the employee never has to let a manager know they are seeing a counsellor.  Nor are reports given by the counsellor on clients she sees.
The council will pay for a 6 session period but this can be extended if need be OR the counsellor will refer onwards if she thinks some needs specialist counselling.

I am interested to know whether those of you who offer counselling services can advise on the following:

How many further appointments are offered to the employee who does not attend on the first, agreed time / date? 

Are there any recommended / documented standards for this?  If so please can you advise on the reference.

For the number of hours availability, how many should be spent on delivery of counselling and what breakdown of other activities are expected?
