I believe that there is a system (form) where HR/Personnel can pass people and that this form is part of the Job Application Form. I understand that this system does not ask for specific any medical information, and questions are worded in such a way that confidentiality is not brokem, and answers are in the form of yes or no. If anyone can point me in the right direction or give me an example of such a form, it would be greatly appreciated.
