The IPD web site is useful and you may find it there.
 
However currently we undertake a risk assessment before individuals are given a contract of employment to assess on ability to undertake role and what adjustments are required both physical and mental.  We would then update HR/Recruitment on suitability for employment and adjustments without breach of confidentially.

Yes I agree, however when it comes to asking functional ability questions for psychological ability it is not quite as clear cut.

I like the approach of asking questions relating to functional ability for each risk presented, so a section on DSE, moving and handling, COSHH, noise, radiation, vibration, past diagnosis with an occupational illness, and whether the individual is on any ongoing medication, creams or inhalers, or has a disability.

Hello

I've recently started at an educational establishment and would like to overhaul Pre Employment Screening.  I believe that there is a system (form) where HR/Personnel can pass people and that this form is part of the Job Application Form. 

I understand that this system does not ask for specific any medical information, and questions are worded in such a way that confidentiality is not brokem, and answers are in the form of yes or no.

If anyone can point me in the right direction or give me an example of such a form, it would be greatly appreciated.
