 Hi John,
 One area is certainly in that of the "Duty of Care" to others and linking this to a professional responsibility to disclose a situation that may expose self/others to risk in Professional courses and/or Placements (fitness to practice issues). Also responsibilities to Support Workers and to updating services of changes in needs or other important changes. 
 New Learning Support and Placement Support Agreements signed by all parties should assist this agreeing of respective responsibilities.

Don't know of one but I'd be delighted to see such a document.  I've
heard "gossip" about such responsibilities and the penalties in other
places.  It does seem appropriate that a student has to make a
commitment of some kind.

lists/discusses/mentions the 'responsibilities' that a student
has under the DDA Part 4 (as opposed to just discussion of
their 'rights'?)
