Several social services departments around the country use this method, not
even asking the individual which category they prefer to be slotted into.

The anonymised method of equal opps monitoring is workable, but have you
tried persuading anyone that has been using the personalised method to change?
They have loads of reasons why it cannot be done and even when you prove that it
can they say - too late now, we've already put the system in place.

One organisation I managed to convince introduced a separate sheet for
monitoring during recruitment but they put a barcode and reference number on the
sheet so that it could be matched with the application form.
