Before moving to the NHS last year, I worked for Thurrock Public Libraries and was in charge of the Mobile Library. We undertook a survey of the service and used the results to analyse various factors - all of which gave us indicators for producing a revised timetable.
I haven't actually got a copy of the questionnaire at work, but I can probably get you one if you're interested.

Hi everyone,

The Special Services, SELB, N Ireland, has 2 housebound mobile library vans serving housebound readers in a predominately rural area.  We are due to carry out a survey of this service in the next couple of months and, as we have never done this before, would welcome any help anyone could offer us in designing the questionnaire.  So, if anyone has carried out anything similar recently and wouldn’t mind passing on any tips you may have, even sample questions/questionnaires, I would appreciate it.
