I have a fairly substantial feedback form in the Learning section of our
website. It didn't need to meet the criteria you mentioned, but it has
proved very useful in building up demographic information and profiles of
the preferences/motivations of our various user groups - this has also been
useful to feed into the prioritising of work plans etc.

When it was being planned I did look at a number of other institutions
feedback facilities, and discovered that they were almost all very simple
open text boxes, but I had a clear need for much more detailed, and
structured information, so we went for the more comprehensive approach which
you'll see on the site. If you have more specific questions I'd be happy to
answer them.

I wonder if anyone has experience or advice on good practice for on-line web
feedback forms?

We are currently undertaking Comprehensive Performance Assessment (CPA)
reviews for an audit that will happen next year and have therefore, as a
part of this, developed a revised set of public survey feedback forms (for
use within the gallery) for aspects of the gallery's work, e.g. exhibitions,
lectures, workshops etc.

We have also identified the need for an online feedback form for the
web-site and/or post gallery visit comments.

Does anyone have any advice or good examples to look at, preferably in a
format that will satisfy CPA guidelines as to accountability for audit
purposes?
