James, I have a fairly substantial feedback form in the Learning section of our website. It didn't need to meet the criteria you mentioned, but it has proved very useful in building up demographic information and profiles of the preferences/motivations of our various user groups - this has also been useful to feed into the prioritising of work plans etc. When it was being planned I did look at a number of other institutions feedback facilities, and discovered that they were almost all very simple open text boxes, but I had a clear need for much more detailed, and structured information, so we went for the more comprehensive approach which you'll see on the site.
