Do all organisations request personal details on these forms?
e.g DoB,name.
If the information is for statistics, surely completing the form and
submitting it with the application form quoting job ref number would be
sufficient.
I dislike the idea of having personal data tied to a form that identifies
the indiviual and contains details of sex, marital status, sexual
orientaion, pyhsical/mental disablity, ethnic origin, colour of skin and
hiv status, or am I being too picky?
