A fresh case from our hospital for discussion/learning:

A 62-year-old lady presented to A/E with being non-specifically unwell. Initial blood tests showed: urea 27.7 mmol/L, creatinine 506 umol/L, sodium 140 mmol/L, potassium 6.7 mmol/L, glucose 9.0 mmol/L, amylase 291 IU/L (< 110). An arterial blood gas analysis showed: pH 6.89, PCO2 3.1 Kpa, PO2 15.2 Kpa and lactate 23.1 mmol/L.

What could it be?
