With difficulty.
Formulae based on urine urea excretion are flawed, because the contribution
by urea to urinary nitrogen excretion can vary between approximately
60-90%, and not predictably. The only sure way is by measuring urine
nitrogen. The problem is compounded in ITU patients in whom changing body
water and plasma urea concentration may be occurring: their contribution
can often only be estimated.
There is extensive literature on this.
