My practice has always been to cross reference in both directions in the
patient's notes. ie to refer to the correction on the original report and
to the original in the correction. Also to check the clinical notes and
correcpondence at the time to see if acted upon. Finally to sign these
comments. If it's a 'big one' to write to the consultant.
