Subject: request restrictions for tumor markers


Dear colleague,

With this e-mail I would like to ask if somebody has information on
what restrictions can be made on repeated testing for tumor
markers, without affecting the quality of healthcare. I would like to
establish tumor marker specific periods in which the test can
normally not be ordered again.

For many test we have implemented these so called "repeat-
periods". If a test is re-ordered within the repeat-period, a warning
message is displayed automatically to the technician entering the
request. At this point the technician denies the request, unless
specific action not to do so has been taken by the doctor or ward.

This mechanism has proven to be useful in preventing the waste of
time and money (about 7000 euro in 2002 on a test-volume of 1
million tests). Therefore, we plan to expand the repeat-periods to
tumor marker tests.

In literature information on this aspect is rare. My best ques until
now is:

HCG: 1 week;
AFP: 1 week;
PSA: 1 month;
rest: 3 months.

Any information, comments or ideas are greatly appreciated.
