On one occasion I remember seeing a very high alk phos on a pregnant lady, at about 37 weeks - can't remember exactly what the level was now, but around 2000, with ULN of 120 u/L. It was virtually all heat-stable, and EP suggested it was all placental ALP.  Levels fell to normal fairly soon after delivery.  Other LFTs were all normal, but bile acids were'nt measured in those days!
