Thank you for the tip. :) Isaac -----Opprinnelig melding----- Fra: RIDDELL, David [mailto:[log in to unmask]] Sendt: 27. september 2001 10:59 Til: [log in to unmask] Emne: Re: Clunk and relocation tests Isaac Take a look at Brukner and Kahn (which I have with me today). It is excellent and although a Sports med book it is written with a strong general outpatient/physio input including rehab e.g.it doesn't stop at 'diagnosis' CLINICAL SPORTS MEDICINE Peter Brukner and Karim Kahn. 2nd ed 2001 McGraw Hill Sydney. Is this book a "must-have"? The relocation test is very helpful in assessing the secondary impingement patient. It is a modification of the apprehension test for anterior instability of the GH joint. The subtle difference is that when in the 90/90 position the patient has more discomfort than the feeling that the shoulder is going to dislocate. At this point you apply a gentle posterior directed force to "relocate" the humerus which results in increased external rotation and decreased discomfort. I find this test positive in the cases of athletes who are the silent subluxors who note complaints of a "dead arm" i.e., no pain but no velocity in the acceleration phase of throwing. The clunk test is supposed to be for a "torn labrum" typically the Bankart lesion. It is in full abduction, patient supine. You place your hand under the shoulder and apply an anterior glide while the other hand externally rotates the shoulder. I find this often times to be false positive. Hope it helps. P.S. Ober test is sidelying assessment of the length of the iliotibial band.
