Sometimes it's just not possible to grasp the context (unless we do serial consultations or can do the psychiatric thing - even then.....) but the main requirement is to try to understand.  Patients really respond to that!

Surely not 'either or'?

I think identifying the simple rules that set the context for the contact
between two people can sometimes help us understand the richness or muddle of
what is going on.

The Driver questionniare (OK very simplistically) looks at five drivers of
behaviour - Please others, Be perfect, Hurry up, Try hard & Be strong.

Maybe self awareness of these context-setting simple rules will help our
understanding of what goes on within this context - it certainly can help us
look at the power relationships within the consultation and why they are as
they are
