We do check the loos in all of the exam venues before the start of the two exam
periods in the day, and students who need to avail themselves of the facilities
are escorted, by an invigilator or porter, into the loo (but not into the
cubicle) if of the same gender.  As has been said short of body searches
there's little more that can be done other than listen for suspicious rustling
noises - then it starts to become silly.  Some invigilators do note on the
attendance sheets which student went out for a loo visit.

Not sure that this helps but short of providing each student with a commode
....
