Dear All,
I would value opinions on an interesting case which presented to us last
weekend.

13 month old boy
H/O possible ingestion of few mouthfuls of methanol 2 hours earlier

On admission:
Na 137, K 4.2, Cl 105, HCO3 24, Urea 5.4, creatinine 367 umol/l(checked!)
serum osmolality 289
glucose 4.8
No increased osmolal gap

MLSO unhappy with creatinine and requested repeat sample (2 hours later)
Na 136, K 4.7, HCO3 24, urea 5.5, creatinine 284

Paediatric registrar could not explain isolated rise in creatinine in
presence of normal urea

12 hours later
Creatinine 201, HCO3 25.8

12 hours later
creatinine 165, HCO3 26.6

Child remained well throughout and was discharged home the following day.

Our creatinines were measured using Olympus "O'Leary" modified Jaffe method.
Initial creatinines (367 and 284 umol/l) rechecked using Vitros "enzymatic"
dry chemistry method and measured 32 and 28 umol/l respectively!!!!.

What could be causing the profound positive interference with the "O'Leary"
modified method or is there another possible explanation?
