I filled in the questionnaire, well part to it. To be frank, I found it offensive! I never heard so much negative medical model self-pity in my life.
I think this is more representative of the researcher's own impairment and the related psychosocial responses.

I am writing an article at the moment on the differing types of impairment, related to psychosocial experiences, rather than biology. I have identified 5 factors including what the impairment occurs, its stability, how long it takes to develop and so on. 

This questionnaire does demonstrate my theory as it is the expected response of someone with a progressive impairment., ie a major lack of confidence and self-esteem. The questionnaire wishes to support his own feelings and is not compatible with all impairments in terms of language and attitude. I would have preferred a more neutral stance.

There were a total of five mistakes in the online survey that was launched
on the 30th of March - this led to a contamination of my data.  I have now
corrected these errors.
I believe the survey is now error free and I am launching it today (Monday
the 3rd of April, still Sunday 2/4 in some places).  I have dumped all of
the data that was collected through the online survey over the previous four
days to protect the integrity of my study.  I am starting afresh to ensure a
clean data file.
I would be most grateful if people who completed the survey before the 3rd
of April could fill it all out again, because your previous responses have
been completely wiped.
Also, if you haven't filled it out, please do so and feel confident that the
survey is now error free.  The survey can be found at:
