Check out Alex Hough's book "Physiotherapy in Respiratory Care" (1996 Stanley Thornes): "Non-alert patients breath shallowly and monotonously and tend to develop patchy atelectasis, which is best pevented by positioning. Neurophysiological facilitation is more short term, but is useful for spontaneously breathing patients who are unable to turn (eg. acute HI). This technique uses tactile and proprioceptive stimulation to increase reflexly the rate and depth of breathing. Examples are..." (Hough 1996). She lists the following ref: Bethune, D.D. (1991). I tried the perioral pressure and the intercostal stretch (on separate occasions) on a guy who was on SIMV on ITU. He had a very rigid chest (I questioned PD but the docs thought not) and his sats weren't all that good. On both occasions, I noted an increase in the chest expansion (spontaneous breathing) and his sats concurrently rose. T'riffic. The nurses thought I'd lost me marbles and asked me if I was a Nazi since I appeared to be doing a Hitler moustache solute on him! They wouldn't believe me when I tried to explain what I was doing. They just thought I'd flipped me lid. Good luck with the search.
