Wow - I've never had so much email in a day!! I'll stop considering suicide now ;-)) There have been as many requests for copies of what I receive as examples. In an attempt to keep bandwidth to a minimum, I'll collate the examples as I receive them, anonymise them to protect the innocent, and then post a url where they can be viewed by anyone interested. Hope this is acceptable to all. So please continue to email your example survey questionnaires directly to me, rather than clog the list. And thanks to all who have responded so far.
