In general this happens with very active kids age 9 -13, boys more often
than girls and occurs bilaterally I about 23 - 30%. It is often seen in
sports with significant amounts of jumping or quick direction changes
(soccer, basketball, track, gymnastics, volleyball)
In the athletes I've seen general treatment includes ice after every
practice or event and often 1 -2 other times a day. I emphasized good
hamstring and quad flexibility. I also would modify practice to eliminate
any portion of practice that was not essential. If the symptoms remain too
sever I held them out of the activity.
I had two athletes who did not come to the training room for treatment or
developed symptoms during the summer when school was not in session. The
first, who received no treatment, actually avulsed his tibial tuberosity and
fractured into the growth plate, while long jumping in practice . The
second was placed in an immobilzer for 4 - 6 weeks. He was able to return
to basketball that fall without any significant problems. the first
required surgery. In general most cases do not end with such drastic
results and tend to be self limiting. Remember to keep the child's age
and goals in consideration when determining a course of action.
