I don't know what document you mean, but it is not true that labels must
be expressed in all uppercase. Thus "Title" is preferred over "TITLE".

No, the mixed case convention in ftp://ftp.isi.edu/in-notes/rfc2413.txt
contains the official recommendation on alphabetic case, which is the same
one adhered to in the draft-kunze-dchtml-00.txt document.
