We at Leicester General have had a weekend service to a limited number of
our cold orthopaedic patients for many years (pre me and I have been here
4years) We provide treatment for minor cases that require mobilisation
inorder to be discharged over the weekend eg arthroscopies done late on
friday, foot surgery etc. and also TKR patients. The appreciation of the
service and its effect on decreasing length of stay enabled us to get extra
funding directly from the orthopaedic directorate, (unlike the respiratory
on call which has to be funded out of the physiotherapy budget) to increase
the number of hours from 2 per day to 4. We also cover the bank holidays
when other patients will be seen also. The service is provided by the
junior physiotherapists and orthopaedic / outpatient staff with senior I
and IIs from medicine and neuro being able to opt on to the rota if they
wish. This means that the burden is shared. It also means that respiratory
on-call staff are not the only ones having to work at weekends ( or the
only ones able to get overtime - depending on your point of view!). I
haven't got round to auditing if there is any difference with having the
service, also it would be difficult to retrieve information from before i
started) but I would be grateful for any info you find out.
Incidentally it means that our TKR average length of stay is approximately
10 days.
