I have seen such a publication called "Dangerous Exercises" produced 1992
or so by the Scottish Sports Council. It was one of a series of small
pamphlets. Other titles included "Infections and Sports" and "Drugs,
Doping and Sports." It was not as comprehensive as the book you describe
but included a wall poster which is on the wall in my gym.
The exercises highlighted included double straight leg raise and circling
the neck. There was a brief explanation about why this exercise was not
safe, and included an alternative that would produce the result without
risk.
