This issue was discussed quite nicely in the recent paper:
AM Smith et al: Investigation of low frequency drift in fmri signal
Neuroimage 9:526-533, 1999.
Why bother having someone hold still, when you can image a cadaver
as this group did?
I highly recommend this paper.
