Dear Craig,
I have to say from the outset that I do not have the article you referred
to; however is this the correlation versus causation problem again? Could
the deformity be caused by something other than the footwear? For
example, it is also plausible that the sedentary nature of office work led
to a decrease in muscle strength in the feet of the office workers and this
casued the deformities. In contrast, the forest workers would have been
walking on uneven ground all day and may have developed exceptional muscle
strength in their feet.
I do not know whether this study was prospective or not, however it could
also be that the office workers had bad feet in the first place and that is
why they gravitated towards office work.
Although, the footwear would appear to be the obvious link, it may not be!

I knew there would be some evidence somewher that footwear does cause
deformity. try this one:
Morioka M et al: Morphological and functional changes of feet and toes of
japanses forestry workers. Journal of human Ergology 3:87-94 1974
They compared foot shape and toe function between forestry workers who wore
a glove like 'jikatabi' with 'seperations' between the toes on their feet
with 'office' workers who did not. The office workers had more foot
deformity.
