I'm working on the Proforma myself - initially I found it helpful because it enables you to structure what you're writing very precisely, which you have to figure out for yourself with the PDR (and I tried, many times - oh, how I tried!).

The Proforma stipulates about 4000 words + appendices, the PDR 4000-6000 + appendices. I don't really think this is part of the difference.

Some parts of the Proforma only ask for ticks or crosses to be placed in boxes (e.g. methods of training received) - these are so that the topics can be discussed at interview.
Although, to be frank, I think PDRs should be subject to interview as well (ducks to avoid flying missiles).
