I have done many, many Bier's blocks in my time, and have seen several
adverse reactions. Now, having said that, the most severe problem I had to
deal with was when an orthopaedic registrar grossly overdosed a very
petite patient with diazepam and pethidine to relocate her shoulder. She
stopped breathing for 20 minutes!
The Bier's block is undoubtedly the best form of analgesia for distal
radial fractures. However, it is time and resource-consuming. We are
grossly under-resourced (I am not proud to say that my Trust is in the 15%
that the Audit Commission found did not increase its medical complement
between 1995 and 1998, despite many pleas from me) and it was simply not a
viable option.
