I have seen a similar case. Male aged 4x who was fit and well the night
before and had 4 or 5 pints with his mates. Found unconscious and
unresponsive the following morning and brought to A&E. On arrival,
hypertonic and fitting with opisthotonus and extensor spasms; left pupil
larger than right. No medication, no recreational drug history known.
Afebrile, no rash. P90, BP 150/74, CS 4 (but fitting) general examination
otherwise unremarkable.
Intubated and ventilated. Bloods unremarkable; urine showed no
metabolites. Gases unremarkable.
