A response from my school. I have used Insieme for the intermediate level
and like it. We have now adopted Crescendo, though, for those classes. I
believe that CUNY/Queens College uses Prego! over 4 semesters.

At Columbia and Barnard use Prego for first year.
Moat of the TAs (who are mostly responsible for teaching elem Ita) do not
really like it, they find it too grammatical and sort of 'fake
communicative'.
Anuway we cover most of it in the first two semesters (but
leave out certain features, such as imperfetto and trapassato subjuntive,
passive, impersonal.)
In intermdiate (3rd and 4th sem) we use, as of this year, Crescendo, which
is full of communicative activities. We do not know yet how it will
perform, we just started.
