At our hospital this is being handled as a "screen" that is handled by the
admitting nurse. Each of the disciplines of rehab along with the nursing got
together and developed the indicators that a patient may or may not need
intervention. It is a relatively basic screen and lists declines in mobility,
safety, or strength or increased pain. The problem is that the physicians do
not want the screening to "dictate" there referrals to rehab. They are afraid
that if it is in the chart, they will be held liable if they do not order the
therapy.
