<<My model for sustainable full-time practice is as follows:
-All OOHs handled by COOP/deputising. Saturdays pro-rata
-Daily visits and availability for telephone advice
-7 surgeries, 2.5 hrs
-1 session for administration and non-GMS work
-1 session free for practice and personal development
-1 session at home
-7 weeks holiday/yr
-Sabbatical every 7yrs

Would tend to go for contracted minimum given present state of GP---ie 42
weeks, 26 hours per week and no personal OOH unless for very good reasons.
Also sabbatical--depends on how long? I hear that of those who take a
year, only aboutt 10% return to "full-time" practice, Is that those who
take it are thinking of leaving anyway or are they so worn out by time they
go that one year of normality makes it impossible to come back or what? I
dunno but for sustainable full-time practice you need to have sabbaticals
with a high rate of return of happy GPs to the coal-face.
7x2.5 hour surgeries sounds good but how often will they run over? And what
mechanisms are there for ensuring they don't run over often? And how about
visits---personally I would think that part of the model should be an
explicit, audited commitment to reducing home daytime visits to the
absolute minimum. (1)
Who covers in between surgeries? That needs pinning down.
Otherwise it sounds good!
