Difficult to work out - nowhere near what you mention though.
It seems to be arbitrary and indeed we had a payment supposedly for
patient care in July to our ex senior partner who retiered 18 months ago
- and as far as I can see none of us had any patients in the hospital at
all at that point.

don't you hate doing that if you've just seen the patient at his own
home, decide he needs nursing care and then they call you to clerk him?
