1. Educate the bastards.
2. Educate the parents (who are a subgroup of the above).

to do all the day time visits on a short term contract (as you'll have
educated all the bastards, so reducing the numbers) and all the rest
do a 1 hour "extras" surgery. Creating vastly more appointments and
keeping the booked surgeries free of extras.
And educate the bastards.
