Part of the problem most doctors have is in drawing the line between what
constitutes a reasonable amount of work, and what is over the top.
You know how they teach you about body language on vocational training
schemes? Well they should also teach you about negative body language, which
I have learned to use when a patient is trying to dump their life in my
in-tray. The trick is avoiding making people dependent on you - because if
this happens you become personally responsible for all their problems and
there are so many of them they just wear you out. The trouble is that this
process takes about ten years, and is so insidious that most people don't
notice it before they are a dried out husk.
We are here to treat medical problems - we are not clerics. Over the years,
I have learnt to give a clear indication to people about the problems I am
not willing to take on. One of my partners does not do this, and although
the patients love him to bits (which is partly why he is so holistic - there
is a short term reward) it is grinding him down at a frightening rate. He
also gets home two hours after I do and never has a lunch break. Don't get
me wrong - I would do what he does if I thought I had the remotest chance of
getting away with it, but I would need a list size of about twenty and
relays of counsellors to keep me straight.
