If we are running independent businesses whose main asset is the
medical skills and persons of the doctors in them then it should not
be surprising that practices demand high rates for these outside
extra jobs, in addition to any payments being made to the practice for
providing GMS in the practice. Not being paid twice, because AFAIK
when a partner goes to the PCG meeting nobody says "so that's half a
day's work less that your practice is expected to provide to
patients"
OTOH if we are some sort of employee with several possible
activities we may do as work, then th question is what should we get
at the end of the year... presuambly net intended annual
remuneration + targets, and the incredible amount of fiddling about
with allowances and claims and people employed just to track them
could be dispensed with. However, somebody would have to find other
doctors to doctor while the doctors were governancing and
administrating and leading.
Decimate the administration and see if efficiency goes down, or if
the survivors evolve new and more effective ways of working.
