The thread on Lawyers has me wondering. Especially the guy who proclaimed

While we can all learn from our own mistakes, it ought to be safer for
everyone concerned to also learn from other peoples. Does medicine have a
system of "near miss" no-blame reporting like pilots or is it just a case of
what makes the papers or the anonymous tips from your insurance company.
And before anyone asks Community Pharmacy doesn't have such a system (though
the hospitals do).
