1. First make sure that any conclusions from the survey are directly
related and interact with the system design. Otherwise don't waste
everybody's time.
2. Get a professional healthcare statistician to do it. Not only to
apply it but also to formulate, design and draw conclusions from it.
Then you might, just might, be playing in the same ball park.
3. Read and digest 'Questionnare Design, Interviewing and Attitude
Measurement' by A. N. Oppenheim; Printer 1992. Then design and print a
questionnare. Rip it up, throw it away and go back to 2.
What will probably happen is that a self-styled researcher will design
the system, build in every bias they enjoy and then slant the findings
to fit in with the cash flow and personal prejudices of the controlling
body and stand back looking cool.
