You shouldn't have to worry about that either.
For example, the GEHR project allows mapping between different codes
and translates between different European languages, and all this is
done transparently.
A doctor in Italy uses ICPC and enters data in Italian. Patient sees
doctor in Portugal, doctor uses ICD10 and enters data in Portuguese.
Patient sees you, you use Read and enter data in English. GEHR would
maintain the stack of information unaltered but 'understandable' by all
3 docs.
