I have an interest in Chronic fatigue syndrome, New Zealand has over
the past two decades been subjected to an increase in Giardiasis, it is
now present in all of our waterways, probably it entered the country
through our tourists. Untreated surface water is highly suspect and
responsible for a large number of cases of giardiasis. Many of these
cases become chronic and our records would suggest that 60%-70% of cfs
sufferers do in fact respond to treatment of Chronic Giardiasis. One of
the clinical markers we use is in fact foul flatus, or in your terms
rotten egg farts and burps. Tinidazole as you have indicated works well
but we find that lower doses over a lengthy period of 2 - 4 weeks is
needed in the chronic form. I would be interested to find in the case
presented if there are more extensive symptoms not mentioned.
Anyhow it appears as if the consensus of opinion is swinging towards
giardiasis.
ps we used to use a follow up course of Naxogin for 2 weeks this very
useful drug is now no longer available in New Zealand. The rational for
this was, in the chronic form of the disease the pathology is related to
the encystation of the trophozoites, metronidazole has about a 40%
cysticidal effectivity where as tinidazole and naxogin have a 95 %
cysticidal action and hopefully not the same 95%, so a complete kill is
hoped for.
