Sorry if I've recounted this one before but we made thin laminated strips to
stick on one side of a monitor with antibiotics grouped not by indication
but cost - green at the top for amoxy / trimethoprim, red at the bottom for
clarithro & ciprofloxacin etc. AB costs fell, no sign of worsening care
3 antibiotic options
prob not needed - nothing is best, cheapest if you must prescribe something
probably needed - cheapest is best (yes amoxy, oxytet, trim)
really needed - whatever you want (unless cheapest will do - eg chron brons
still get amoxy first line)
Prescribe a quinolone about once a quarter I guess?
I recently compared the ratio of cardiovascular to antibiotic prescribing
costs for a group of local practices - interesting comparator I reckon -
both groups are more likely to be used in high morbidity / need practices
so may reflect physician prefrence rather than case mix?

in next few weeks I am facilitating a GP practice reviewing their own
prescribing of antibiotics
we are compiling a practice formulary
what issues would you review , audit etc
I will ask
when do they consider an antibiotic is NOT needed ?
can amoxicillin be usual 1st choice , in uncomplicated URTI
no local resistance , IF ab required
UTI- trimethoprim ?
do you see a place for quinolones in primary care ?
