If there's anything embarassing or potentially harmful to the patient I
ask them about recording it and respond to their wishes. Their choice.
Someone suggested a sign in the surgery advising patients to ask for
such if required. You could always record the patient's request rather
than the actual data for medico-legal purposes.
I consider statistics gathering as a side effect of the patient's
illness, i.e. of secondary importance.
