I have a partial solution for this one. I have been alpha-testing the
"Clinergy" front-end software (formerly known as PEN & PAD) for Meditel
System5. It makes entering clinical data much quicker and easier, and in
particular uses qualifiers such as present, absent, possible and excluded.
Admittedly, they are then coded as 9NZ, but at least I have a defensible
record, and the free text terms are entered consistently. i don't think we
can do much more until they revamp the Read codes
