Absolutely!!! The problem is with the structure, the system, the
organisation. The early pioneer of TQM (Deming) I believe said exactly
the same thing and then introduced it to Japan but that's another story.
OUr present situation is that we have a hidebound GP admin system where
umpteen regulations are produced, introduced and written down. It is an
unwieldy beast but it never occurs to either the beast or the beast's
minders to put the beast on the pill---ie stop it reproducing itself. If
you can get you should J. Philip Lathrop's book "Patient-centered care---a
new paradigm" from Joosey-Bass publishers (San Francisco I think) in which
he explains wonderfully just why health service/institution/hospital
regulations and organisations grow so completely out of control and so out
of touch with the needs of both patients and workers.
