I admit I was thinking primarily of GPs but the principle of motivation
and reward should be applicable elsewhere, wherever endeavour exceeds
expectation. Although money may not be an appropriate motivational
factor everywhere, its perceived inadequacy is most certainly
demotivational.
Apart from financial rewards, there are many other motivating factors,
perhaps most of them stated succinctly in Maslow's Needs, some of which
may be also applied to renumeration.

Whatever is decided by peers and the paymasters. I have my own ideas but
the real deal should come from a consensus.
The gov's present suggestion is politically self-serving insofar as it
wants to reward commitment to gov. policy. This is such an ephemeral
thing that it should be carefully examined before being professionally
accepted. That does not disqualify the principle guiding it, however.

Nope. More a triumph of hope over rationality. However, an incremental
'loyalty' bonus may be of some benefit. A C grade may well do this. The
As and Bs etc would be restricted to 'achievement'.

Only in a sensual context. Otherwise it is too dependant upon the
individual's personal reality to be of much use to society or the group.
I was thinking of GPs because once one becomes such, there is no further
development or heirarchy for which to aim for, unlike academic medicine
or the nursing profession. Many GPs take to other subjects for
fulfillment, such as IT or business or further degrees etc. Also there
is no external incentive to be any better than on the day of becoming a
partner, thus promoting stagnation and the inevitable loss of morale. To
promote excellence requires motivation and that may not always be
available on a personal basis.
