We have been using my program for over a year now.
Initially it all went very well!
Then things started to go wrong. Why?
Looking at those patients with poor control, it soon became apparent
to me that most problems were in those requiring a high target INR
and these patients were also in receipt of polypharmacy which was
altered frequently, by us and by the hospitals.
The other main reason appears to be alcohol. Actually had to "rescue"
one chap from his local watering-hole before he started to bleed to
death.
Are our results better than the hospitals?
We deal with three local DGH's :-(
Only one, YES ONLY ONE, has ever audited it's warfarin clinic. Better
results than us, but especially significantly so.
Before panicing, look at the control your patients had before
starting the practice clinic.
Starting off new patients is bound to throw your figures out.
NEVER START A PRACTICE CLINIC WITHOUT NEW FUNDS FROM YOUR HA.
